Information for patients
PR Renal and Research accept most of the health insurances including Vital. Some health insurance requires referrals to receive our services. For more information or for appointments in one of our clinics near you, contact us from Monday to Friday from 7:30am – 4:30pm.
Medical record
PR Renal and Research is required to provide you with copies of your medical records related to care and treatment at any of our facilities. To receive copies of your medical record, you must complete the document REQUEST FOR A COPY OF MEDICAL RECORD and deliver it to the clinic where you receive services.
